Contract Requirement:

• Assignment must continue into July
• Earliest end date: July 6
• Duration: 26 weeks

Schedule & Staffing

• Floating Between Units: Yes, Candidate must be willing to float up to 1 day per week to Providence Willamette Falls or Providence Milwaukie, based on staffing needs.

   

• Block Scheduling: Required

   

Experience & Requirements

• Years of Experience: Minimum 2 years

• Required Experience:

  • 2 years of Case Management experience

   

Licensure & Certifications

• LCSW – Required (License in hand)
• State license required
• Oregon RN license required if candidate is a nurse

Charting System

• EPIC – Required
