Job Title: Case Manager (Discharge Planner LVN)

Job Requirements & Qualifications:

Certifications:
- Required: LVN / LPN
- Preferred: Case Management certifications (not specified, but could be beneficial)
Experience:
- 2 years in an adult acute care hospital setting in Case Management.
- Acute Care Hospital Case Experience is a hardstop requirement (Behavioral Health and Outpatient experience does not count).
- Charting System: Experience with Epic is required.
Skills:
- Acute Hospital, Admission Criteria, Care Coordination, Discharge Planning, DRG (Diagnosis Related Groups), Prior Authorizations, ICU, Surgical Services, Needs Assessment, and more.
- Knowledge of CMS, HIPAA, The Joint Commission, and National Safety Goals.

Unit & Work Details: Unit: Discharge Planner LVN (Unit Code: 2910.861060.27505) Patient Age Groups: Adults Patient Care: Discharge planning with a case load of up to 50 discharges, with higher numbers over weekends. Interdisciplinary Support: Includes Physical Therapy, Social Services, Transportation, and Unit Secretary. Weekend Coverage: Required, with the number of weekend shifts per contract being flexible.
