Licensed Master Social Worker (LMSW) – Hybrid

Location: New York, NY (Hybrid – Remote with in-office presence once per month as needed)
Schedule: Flexible hours (No 10-hour shifts)
 

Job Overview

We are seeking an experienced and compassionate Licensed Master Social Worker (LMSW) to join our team in a hybrid capacity. This role offers flexibility, autonomy, and the opportunity to collaborate with a multidisciplinary care team while providing high-quality social work services to a diverse client population.
 

Responsibilities

• Conduct comprehensive psychosocial assessments
• Develop and implement individualized care plans
• Provide counseling, case management, and support services
• Collaborate with healthcare providers and social service professionals to coordinate care
• Maintain accurate, timely, and compliant clinical documentation
• Participate in ongoing professional development and apply evidence-based practices

Requirements

• Active New York LMSW license (and license in state of residence, if applicable)
• Medicare and Medicaid enrollment required
• Minimum 1 year of LMSW experience (no new graduates)
• Spanish-speaking strongly preferred
• Ability to attend in-office work once per month if required
• Comfortable working in a hybrid (remote/in-person) environment
