Description

Job Title: Home Health Registered Nurse (RN)
Location: Valley Health System
Start Date: 01/05/2026
Contract: 13 weeks
Schedule: Day shift, 5×8s (8:00 AM – 4:30 PM)
Pay: $72/hr ($2,911/week)

Job Description

Provide skilled nursing care to patients in their homes in accordance with physician orders and care plans.
Perform comprehensive patient assessments and monitor ongoing health status.
Administer medications, treatments, wound care, and other skilled nursing services.
Educate patients and caregivers on disease management, medications, and home care needs.
Accurately document visits, assessments, and care provided in a timely manner.
Communicate effectively with physicians, case managers, and interdisciplinary team members.
Ensure compliance with agency policies, state regulations, and patient safety standards.

Requirements

Active Registered Nurse (RN) license (state-compliant).
Previous Home Health nursing experience preferred.
Strong assessment, clinical judgment, and independent decision-making skills.
Reliable transportation for patient home visits.
Excellent documentation and communication skills.
