Position Overview
We are seeking a qualified Speech-Language Pathologist (SLP) for a contract assignment in a Skilled Nursing Facility (SNF) setting. The SLP will evaluate, diagnose, and treat patients with speech, language, cognitive, and swallowing disorders, collaborating closely with the interdisciplinary rehabilitation team to support optimal patient outcomes.
Key Responsibilities
- Conduct comprehensive speech, language, cognitive, and dysphagia evaluations
- Develop and implement individualized treatment plans in accordance with physician orders
- Provide therapeutic interventions to improve communication and swallowing function
- Accurately document evaluations, treatments, and progress using the designated EMR system
- Collaborate with nursing staff, physicians, and rehabilitation team members
- Maintain compliance with all state, federal, and facility regulations
Requirements
- Active and unrestricted California SLP license
- Minimum of 1 year of recent SNF experience
- Working knowledge of Rehab Optima (Net Health)
- Strong clinical, communication, and documentation skills
- Ability to work independently and as part of a multidisciplinary team
