Position Details:
• Schedule: 40 hours/week
o Preferred: Monday–Friday, 8-hour days (5x8)
o Ability to work holidays a huge plus 
• Call: 2–4 weekday nights per week
• Weekends: 1–3 weekends over the course of the 13-week contract
• Contract Length: 13 weeks 
• Start Date: ASAP
Requirements:
• Minimum of 2–3 years of Home Health or related experience
• Hospice experience preferred; training will be provided if needed
• BLS and ACLS certifications required
Additional Notes:
• All care is community-based; we do not provide services in assisted living settings
• Inpatient support is limited to our Med/Surg floor as needed
• Typical caseload:
o 4–5 patients per day for 8-hour shifts
o 5–7 patients per day for 10-hour shifts
