Administering medication to patients.
Creating reports to track the patients' progress.
Working with other professionals to create treatment plans.
Cleaning and re-dressing wounds if needed.
Measuring vital signs.
Monitoring changes in behavior and progress.
Perform comprehensive patient assessments, including physical, psychosocial, and environmental evaluations.
Develop and implement individualized Plans of Care (POC) in collaboration with the physician and interdisciplinary team.
Provide skilled nursing services, such as wound care, IV therapy, medication management, catheter care, and chronic disease monitoring.
Educate patients and caregivers on disease management, medication regimens, and home safety to promote independence.
Accurately document all clinical visits and communications in HCHB EMR systems in a timely manner.
Communicate changes in patient condition promptly to physicians and obtain/implement new orders as necessary.
Collaborate with therapists, social workers, home health aides, and other care providers to ensure coordinated care.
Maintain compliance with Medicare, state, and agency regulations, infection control policies, and safety standards.
Participate in case conferences, quality assurance programs (QAPI), and continuing education activities.

Must have CA State RN license Must have BLS AHA Must have Home Health experience with HCHB EMR.
