Description

Part-Time | 2–3 Days per Week | 8:30 AM – 5:00 PM
On-Site / Community-Based Role

Provide nursing care to PACE participants in their homes, community, and clinic settings
Conduct comprehensive nursing assessments and triage participant needs
Collaborate with an interdisciplinary care team to create and manage individualized care plans
Educate participants and families on chronic and acute health conditions
Communicate changes in participant condition and coordinate transitions of care
Maintain accurate electronic documentation and clinical records
Participate in quality improvement initiatives
