Registered Nurse – NYIA Assessor
(Ongoing Position, No End Date | Hybrid/Remote | Statewide: Downstate & Upstate NY)
We are hiring experienced Registered Nurses for ongoing, long-term NYIA Assessment positions with no set end date. This hybrid role includes both field and virtual assessments and is available across New York State, including Downstate (NYC, Long Island, Westchester) and Upstate regions.
Position Overview:
As a NYIA RN Assessor, you will conduct comprehensive in-person and virtual assessments to determine eligibility for community-based long-term care services. You will utilize the UAS-NY tool to assess medical, functional, and psychosocial needs, ensuring accurate, compliant documentation.
Minimum Requirements:
• Active RN license in New York State
• 2+ years RN experience in home care, assessments, or community health
• Experience with the UAS-NY Community Assessment Tool
• Valid New York State–issued ID
• Strong clinical assessment and documentation skills
• Ability to travel for field assessments (statewide)
