<ul><li>Functions as the clinical resource for patients and staff for ostomy, Continence Disorders, Acute and Chronic Wound and Skin breakdown and Fistulae.  </li><li>Provides direct wound and ostomy care in the inpatient and outpatient setting. </li><li>Provides patient/family teaching and instruction of wound and ostomy management both pre and post operatively. </li><li>Assists the healthcare team in the evaluation of skin and wound products and equipment along with providing educational programs, in-services and bedside teaching for the nursing staff.  </li><li>Stays up to date with skin care practices and shares advances in wound and skin care assessments, evaluations, treatments and prevention measures. Assesses all patients with a pressure injury staged at a 3 or above and sees all patients that have developed a hospital acquired pressure inury (HAPI) and meets appropriate reporting requirements. </li><li>Assesses overall hospital needs and practices to eliminate HAPIs.   </li></ul>
