Description
LPN with at least 2 years of hospice experience, excellent documentation skills and learns EMRs quickly; may be required to be on-call.
Must be compassionate and works well with others Standard hours- 9am-5pm, Monday - Friday, coordinate with RNs to ensure patients are seen at home..
Rotate with other staff for on call shifts evenings (5pm-9am) or weekends, make visits as requested by triage nurses..
Charting: Netsmart..
Any color scrubs allowed..
Flu vaccine required..
HCP will get a fuel card but until they receive the card, they can submit receipts for 2 fill ups per week, up to $75 per fill up (these are the same limits set on the fuel card for all our staff in Denver)
