Job Description

North Country Home Health & Hospice Agency is seeking an experienced Physical Therapist to provide high-quality home health and hospice services across its northern New Hampshire territory, ranging from Groveton to Pittsburg, NH. This role requires independent travel, including winter driving conditions, and a strong background in home-based patient care.

Responsibilities

• Provide physical therapy services to patients in their homes across pediatric through geriatric populations
• Perform evaluations, develop care plans, and implement treatment programs
• Complete accurate and timely documentation, including OASIS assessments
• Collaborate with interdisciplinary home health and hospice teams
• Educate patients and caregivers on therapeutic exercises and safety
• Ensure compliance with agency policies, state regulations, and best practices

Required Qualifications

• Minimum 2 years of current Physical Therapy experience in Home Health
• Home Health and/or Hospice experience
• OASIS experience
• Experience treating patients across Pediatric to Geriatric age groups
• Active Physical Therapist license (must be current at time of submission)
• Active American Heart Association (AHA) BLS certification
• Ability to work independently and manage a mobile caseload
• Must have own reliable vehicle and be comfortable driving in winter weather conditions
