Job Title: LPN Home Health

Job Description:

We are looking for a compassionate LPN with an ability to remain calm under pressure. The LPN will be responsible for monitoring patients, performing routine checks, and assisting doctors and nurses.

To be successful as an LPN you should be able to take direction and work well in stressful situations. A good LPN has great observational skills and empathy for patients while remaining professional.

Responsibilities:

Provide direct patient care, including administering medications, wound care, and other nursing interventions as prescribed by physicians.

Monitoring patients’ overall health during every visit.

Changing bandages, wound dressings, and catheters.

Setting up exercise programs for patients.

Provide patient and family education on various health topics, including medication instructions, self-care, and disease management.

Explaining procedures and treatment protocols to residents.

Supervise nursing assistants and other support staff, ensuring adherence to established protocols and high-quality care.

Schedule:

Shift time: DAYS - 8 HR: Monday - Friday

Contract: 13 Weeks

Productivity expectation is 7 points per day Traveler is expected to have their own transportation. South territory (South Santa Rosa, Petaluma, Rohnert Park).
