Description
Specialty: Physical Therapy Assistant (Outpatient)
Job Description:
- Collaborates with the interdisciplinary team, the patient/family, and the patient’s primary care provider to develop a holistic plan of care focused on achieving the patient/family physical therapy goals.
- Instructs, coaches, guides, and intervenes to support the patient and family in achieving the best possible outcomes while remaining in the communities they value..
- Assesses the patient and collaborates with the interdisciplinary team and primary care provider to develop the patient-centered plan of care.
- Provides physical therapy treatment to the patient/family/caregiver as ordered.
- Completes accurate clinical documentation to effectively support service provided within specified timeframes
- Provides physical therapy to pediatric, adolescent, adult and geriatric patients based on principles of growth and development and life stages.
- Provides instruction to the patient/caregiver and revises the treatment plan as appropriate.
- Maintains communication regarding patients with the case manager and other disciplines via care conferences or other appropriate methods.
- Reports acute medical problems and condition changes to the primary care provider.
- Completes regulatory required assessments within specified timeframes.
- Completes medication reconciliation at specified timeframes.
Qualifications:
- At least two (2) years of physical therapy experience highly preferred.
- Current license in Physical Therapy or ability to obtain prior to hire.
- American Heart Association HealthCare Provider BLS preferred upon hire, and required within 90 days of hire.
- Excellent customer service skills
- Ability to work without direct supervision.
- Provides care appropriate to the age of patients served on the job.
- Knowledge of physical therapy assessment and treatment methods
- Knowledge of physical therapy modalities and equipment
- Ability to maintain effective relationships with other health care professionals
- Must be able to develop skills in use of technology used in home care setting.
- Demonstrate the highest level of professionalism when interacting with patients, families, physicians, and hospital staff members.
