Description

Submission Requirements

Provider must have 24 months minimum of current Radiology/X-Ray Tech experience - [REQUIRED]
Certifications must be American Heart Association or American Red Cross - [REQUIRED]
Verification cannot show any hits on license at time of submission - [REQUIRED] [DOCUMENT]
Banner will not approve RTO within the first week or holidays. Can not exceed 7 days - [REQUIRED]
Flu vaccination - Banner declinations can be applied for (reach out for forms) **Please add vaccination status in the submission notes**

Certification Requirements ARRT BLS (AHA) AZ Radiologic Technologist license
