Description

Requirements:

Candidate is expected to have reliable transportation and no additional scheduling restrictions. All travel pairs will be required to have their own transportation.
Department Details & Patient Diagnoses: Serving adults 55 and older who are low income, medically complex, have mental dx, behaviors, and/or substance abuse.
Traveler must be willing / interested in joining facility permanently after their assignment.
EPIC EMR experience is REQUIRED.
License/Certs REQ: BLS, LMSW
Years of Experience REQ: At least 1 YR of experience working with geriatrics.
First-Timers Accepted: Yes
Locals Accepted: Yes
