Job Title

Therapist - Physical Therapist

Location

Gearhart, OR

Schedule / Job Type

Full-time, day shifts, 8 hours per day, Monday through Friday

Key Responsibilities

Provide skilled physical therapy services to home health patients in the designated area
Conduct patient assessments and develop individualized treatment plans
Document patient progress using EPIC and Rover applications
Travel within the local service area, with minimal drive time between visits
Collaborate with healthcare team members to ensure comprehensive patient care
Maintain compliance with all facility and regulatory requirements, including COVID vaccination and boosters

Required Qualifications

Minimum of 1 year of skilled home health experience
Valid and pending state license accepted
BCLS certification required
Reliable personal transportation
Ability to work independently and manage a caseload effectively

Preferred Experience / Skills

Experience with Oasis documentation
Familiarity with home health care systems and protocols
Ability to meet productivity targets (standard 30 points)
