Description

Shift:
Full-time, Day Shift

Work Experience:
Three to five (3–5) years of hospital social work experience preferred
Experience developing post-acute care and discharge plans for high-risk patients
Experience with crisis intervention, psychosocial assessments, and community resource coordination
Experience assisting patients with eligibility determination for social programs, funding sources, and in-home care services

Education:
Bachelor’s degree in social work (required)
Master’s degree in social work (preferred)

License:
California Licensed Clinical Social Worker (LCSW) – Required

Certifications:
BLS/CPR Certification
