Description
Pride Health is hiring Home Health RNs to support our client in Denver, CO 80221. This is a 3+Months assignment (Possibility of extension or conversion). opportunity and a great way to start working with a top-tier healthcare organization!
Position Summary:
Primary functions of the Registered Nurse are to administer skilled nursing care for clients of all ages in their place of residence, coordinate care with the interdisciplinary team, patient/family and referring agency and assume the responsibility for coordination of care.
Essential Duties and Responsibilities:
- Assess and monitor home health or hospice patients, developing and updating individualized Plans of Care based on physical, psychosocial, and environmental needs.
- Maintain accurate and timely documentation of patient care, ensuring quality, continuity, and compliance with regulatory and third-party payer requirements.
- Educate patients and families on medications, care techniques, safety, and self-care based on age, condition, and readiness to learn.
- Supervise and guide support staff (HHA/STNA/LVN) and coordinate referrals to other healthcare agencies as needed.
- Perform on-call duties, participate in interdisciplinary team meetings, and communicate effectively with all parties involved in patient care.
Required Knowledge, Skills & Abilities:
- Effectively assesses and responds to patient and family needs in diverse care settings.
- Demonstrates strong communication, organizational, and critical nursing skills, including wound care, phlebotomy, and IV therapy.
- Competent in using and maintaining medical equipment, catheters, and administering respiratory therapy.
- Maintains current licensure, confidentiality, and meets continuing education requirements.
- Works independently and collaboratively, adapting to flexible schedules and local travel.
Required Qualifications:
License/Certification:
- Current state license as a Registered Nurse - Required
- CHPN certification - Preferred
- Current BLS certification - Required
- Current driver's license - Required
- Must have reliable transportation and insurance - Required
Experience:
- Homecare Homebase Software Experience - Required
- OASIS Experience - Required
- 1-2 years' experience as a Registered Nurse in a clinical care setting - Required
- 2+ years' Home health experience - Required.
Education:
- Graduate of an accredited Associate Degree in Nursing or Nursing Diploma program - Required
- Bachelors of Science in Nursing - Preferred
