Physical Therapist (PT) – Home Health | Meyersdale, PA

MedAdventures is seeking a skilled Physical Therapist to join a home health team in Meyersdale, Pennsylvania. In this role, you’ll provide in-home care to patients, helping them improve mobility, strength, and independence while working closely with nurses and other rehab professionals.

Your Responsibilities:

Provide high-quality PT care in patients’ homes
Complete OASIS assessments and develop individualized care plans
Collaborate with nurses, therapists, and interdisciplinary team members
Maintain accurate, timely, and compliant documentation
Educate patients and caregivers on exercises, safety, and recovery

Qualifications:

Active Pennsylvania PT license
Home health experience required, including OASIS documentation
Strong clinical reasoning, communication, and teamwork skills
