Physical Therapist (PT) – Skilled Nursing Facility | Lincoln, MA

MedAdventures is seeking a skilled Physical Therapist to join a skilled nursing facility in Lincoln, Massachusetts. In this role, you’ll provide therapy to residents to improve mobility, strength, balance, and functional independence while collaborating with nurses and the interdisciplinary rehab team.

Your Responsibilities:

• Provide high-quality PT services to residents in a skilled nursing setting
• Conduct evaluations and develop individualized treatment plans
• Collaborate with nurses, therapists, and other care team members
• Maintain accurate, timely, and compliant documentation
• Educate residents and staff on mobility, exercises, and safety strategies

Qualifications:

• Active Massachusetts PT license
• SNF experience preferred
• Strong clinical reasoning, communication, and teamwork skills
