🩺 Role Overview
The Women’s Health High-Risk Nurse Navigator functions under nursing leadership and serves as a clinical expert, educator, advocate, and liaison for high-risk women’s health patients.
This role supports the development, implementation, and maintenance of high-risk women’s health programs, including cardiac obstetrics, while ensuring compliance with regulatory standards, tracking outcomes, and leading performance improvement initiatives.
🔹 Key Responsibilities
Patient Care & Case Management
- Coordinate patient care across multidisciplinary teams
- Assist patients with appointment scheduling, referrals, and follow-up
- Support medication refills, prior authorizations, and medical equipment needs
- Manage individual patient cases and guide optimal care pathways
- Advocate for patients to ensure access to appropriate services and follow-up
- Provide accurate, timely documentation and communication
Clinical Expertise & Program Support
- Serve as a clinical expert and resource for women’s health and high-risk OB care
- Support development and maintenance of high-risk women’s health programs
- Assist in creating and monitoring clinical practice guidelines
- Evaluate outcomes against national standards and best practices
- Analyze clinical data and contribute to quality improvement initiatives
Quality, Compliance & Performance Improvement
- Monitor compliance with regulatory, accreditation, and registry requirements
- Support Joint Commission and regulatory reporting requirements
- Participate in performance improvement and quality management initiatives
- Track patient outcomes and program effectiveness
Patient & Family Advocacy
- Serve as a primary educational resource for patients and families
- Communicate effectively with patients, families, physicians, and staff
- Promote patient- and family-centered care to enhance satisfaction and outcomes
🎓 Education
- Required: Associate Degree or Diploma in Nursing
- Preferred: Bachelor of Science in Nursing (BSN)
📜 Licensure & Certifications
(Required upon hire unless otherwise stated)
- Active RN License (Single-State or Compact)
- BLS – Basic Life Support
🧠 Experience Requirements
- 2+ years experience in cardiology or obstetrical nursing (required)
- 5+ years experience in cardiology or obstetrical nursing (preferred)
