Job Summary

We are seeking an experienced social worker to support patients and families in an acute care hospital setting. The social worker will play a key role in psychosocial assessments, crisis intervention, discharge planning, and coordination of community resources while collaborating closely with interdisciplinary healthcare teams.

Roles and Responsibilities

Perform comprehensive psychosocial assessments for patients and families
Provide crisis intervention, counseling, and emotional support in acute and trauma care settings
Collaborate with physicians, nurses, case managers, and interdisciplinary teams to support patient care plans
Assist with discharge planning, including referrals to post-acute care and community resources
Advocate for patients to ensure continuity of care and access to appropriate services
Maintain accurate, timely documentation in the electronic health record
Participate in multidisciplinary rounds and care conferences
Ensure compliance with hospital policies, ethical standards, and regulatory requirements

Skills

Strong understanding of hospital-based and acute care social work
Excellent communication, interpersonal, and crisis management skills
Ability to work independently in a fast-paced environment
Proficiency with EPIC or similar electronic medical record systems
Strong organizational and documentation abilities

Requirements

Minimum 2 years of experience as a Social Worker in an acute or hospital setting
Active license eligible for submission
BLS Certification (required)
Bachelor’s degree in Social Work or related field
Experience working in Trauma Level III facilities preferred
