Shift & Schedule

- Schedule: 4 x 10-hour shifts

Job Summary
We are seeking an experienced social worker to support patients and families in an acute care hospital setting. The social worker will play a key role in psychosocial assessments, crisis intervention, discharge planning, and coordination of community resources while collaborating closely with interdisciplinary healthcare teams.
Roles and Responsibilities
- Perform comprehensive psychosocial assessments for patients and families
- Provide crisis intervention, counseling, and emotional support in acute and trauma care settings
- Collaborate with physicians, nurses, case managers, and interdisciplinary teams to support patient care plans
- Assist with discharge planning, including referrals to post-acute care and community resources
- Advocate for patients to ensure continuity of care and access to appropriate services
- Maintain accurate, timely documentation in the electronic health record
- Participate in multidisciplinary rounds and care conferences
- Ensure compliance with hospital policies, ethical standards, and regulatory requirements
Skills
- Strong understanding of hospital-based and acute care social work
- Excellent communication, interpersonal, and crisis management skills
- Ability to work independently in a fast-paced environment
- Proficiency with EPIC or similar electronic medical record systems
- Strong organizational and documentation abilities
Requirements
- Minimum 2 years of experience as a Social Worker in an acute or hospital setting
- Active license eligible for submission
- BLS Certification (required)
- Bachelor’s degree in Social Work or related field
- Experience working in Trauma Level III facilities preferred
