Physical Therapist (PT) – Skilled Nursing Facility | Barrington, IL

MedAdventures is seeking a skilled Physical Therapist to join a skilled nursing facility in Barrington, Illinois. In this role, you’ll help patients improve mobility, strength, and independence while collaborating with the interdisciplinary rehab team.

Your Responsibilities:

Evaluate patients and develop individualized treatment plans
Provide high-quality PT interventions to improve functional mobility
Collaborate with PTAs, nurses, and other rehab staff
Maintain accurate, timely, and compliant documentation
Educate patients, families, and staff on mobility, safety, and discharge planning

Qualifications:

Active Illinois Physical Therapist license
SNF experience preferred
Strong clinical reasoning, communication, and teamwork skills
Commitment to delivering patient-centered, compassionate care
