Setting: Skilled Nursing Facility (SNF) / Inpatient Rehabilitation with occasional Outpatient coverage

Position Overview

We are seeking a licensed Physical Therapist (PT) for a contract assignment primarily supporting Skilled Nursing Facility (SNF) and inpatient rehabilitation services, with some outpatient coverage as needed. This role requires flexibility and strong clinical skills to treat adult patients across multiple care settings.

Under general supervision, the Physical Therapist performs evaluations, develops and implements individualized treatment plans, and provides comprehensive therapy services focused on habilitation and rehabilitation.

Key Responsibilities

• Perform diagnostic assessments and create individualized physical therapy treatment plans
• Deliver therapy services primarily in SNF and inpatient settings, with occasional outpatient coverage
• Treat adult patients with a wide range of functional and mobility needs
• Educate patients and caregivers on therapeutic exercises and recovery strategies
• Maintain accurate and timely documentation in the electronic medical record
• Collaborate with interdisciplinary healthcare teams to support patient outcomes
• Provide coverage across shifts, weekends, and holidays as required by departmental operations

Equipment & Systems

• Meditech electronic medical record (EMR)

Required Qualifications

• Active Hawaii Physical Therapist license required by assignment start
  • Candidates without an active license must apply upon offer acceptance
• Minimum 6 months of experience working with adult patients in at least one of the following settings:
  • Acute Care
  • Skilled Nursing Facility (SNF)
  • Rehabilitation Hospital
  • Outpatient Therapy
• Strong clinical judgment and adaptability across care environments

Certifications

• BLS/BCLS required

Schedule Details

• Department provides 7-day coverage
• Typical schedule includes 5 working days per week
• Possible schedules include Sunday–Thursday or Tuesday–Saturday

Orientation & Training

• Department and EMR orientation provided

Additional Requirements

• Must arrive the Thursday prior to assignment start to complete employee health requirements on Friday
• Bilingual skills not required
