Job Description: Occupational Therapist (OT) – Skilled Nursing Facility | Contract

Setting: Skilled Nursing Facility (SNF)

Position Overview

We are seeking an experienced Occupational Therapist (OT) for a contract assignment in a Skilled Nursing Facility. The Occupational Therapist will evaluate, plan, and implement individualized therapy programs to help residents improve functional independence and quality of life.

Key Responsibilities

  • Perform occupational therapy evaluations and develop individualized treatment plans
  • Provide therapeutic interventions focused on ADLs, functional mobility, strength, and coordination
  • Monitor patient progress and modify treatment plans as needed
  • Educate patients and caregivers on adaptive techniques and equipment
  • Collaborate with interdisciplinary care teams to support resident outcomes
  • Maintain accurate and timely documentation in the electronic medical record
  • Ensure patient safety and compliance with rehabilitation standards

Required Qualifications

  • Active California Occupational Therapist license (must be current)
  • Minimum 1 year of Occupational Therapy experience in a Skilled Nursing Facility
  • Proficiency with NetHealth / Rehab Optima documentation system required
  • Strong clinical, communication, and time-management skills
