Job Description: Physical Therapist (PT) – Skilled Nursing Facility | Contract

Position Overview

We are seeking a licensed Physical Therapist (PT) for a contract assignment in a Skilled Nursing Facility. The Physical Therapist will evaluate patients, develop individualized treatment plans, and deliver therapy services focused on improving mobility, strength, and functional independence.

Key Responsibilities

  • Perform physical therapy evaluations and establish individualized plans of care
  • Provide therapeutic interventions to improve patient mobility, balance, strength, and endurance
  • Monitor patient progress and adjust treatment plans as appropriate
  • Educate patients and caregivers on exercises, safety, and mobility techniques
  • Collaborate with interdisciplinary healthcare teams to support patient outcomes
  • Maintain accurate and timely documentation in the electronic medical record
  • Ensure compliance with facility policies, rehabilitation standards, and patient safety guidelines

Required Qualifications

  • Active California Physical Therapist license (must be current)
  • Minimum 1 year of Physical Therapy experience preferred
  • Skilled Nursing Facility (SNF) experience preferred
  • Familiarity with NetHealth / Rehab Optima documentation system preferred
  • Strong clinical judgment, communication, and time-management skills

Additional Details

  • Bilingual skills not required
Candidates must meet the minimum qualifications and hold the required certifications to be eligible for application.
