Position Overview

We are seeking a licensed Physical Therapist (PT) for a contract assignment in a Skilled Nursing Facility. The Physical Therapist will evaluate patients, develop individualized treatment plans, and deliver therapy services focused on improving mobility, strength, and functional independence.

Key Responsibilities

Perform physical therapy evaluations and establish individualized plans of care
Provide therapeutic interventions to improve patient mobility, balance, strength, and endurance
Monitor patient progress and adjust treatment plans as appropriate
Educate patients and caregivers on exercises, safety, and mobility techniques
Collaborate with interdisciplinary healthcare teams to support patient outcomes
Maintain accurate and timely documentation in the electronic medical record
Ensure compliance with facility policies, rehabilitation standards, and patient safety guidelines

Required Qualifications

Active California Physical Therapist license (must be current)
Minimum 1 year of Physical Therapy experience preferred
Skilled Nursing Facility (SNF) experience preferred
Familiarity with NetHealth / Rehab Optima documentation system preferred
Strong clinical judgment, communication, and time-management skills

Additional Details

Bilingual skills not required

Candidates must meet the minimum qualifications and hold the required certifications to be eligible for application.
