Description

Amergis is actively searching for a local or contractor Physical Therapist for a contract, possible temp to permanent opportunity in the Modesto, CA area. Apply today to learn more!

The Physical Therapist is responsible for performing patient evaluations, as well as providing and documenting therapy services in accordance with the plan of care developed for each individual patient and physician’s orders when applicable.

Minimum Requirements:

Current licensure as a Physical Therapist in the state of practice
Graduate of an accredited school of Physical Therapy
One (1) year of prior professional Physical Therapy experience preferred
Current CPR if applicable
TB Questionnaire, PPD or chest x-ray if applicable
Current Health certificate (per contract or state regulation)
Must meet all federal, state and local requirements
Must be at least 18 years of age

Work Schedule: Monday-Friday, 8am-4:30pm Clients ages: Birth to 21 years Tasks: Caseload is approximately 6 clients per day Provide physical/occupational therapy evaluations and appropriate treatments Design treatment plans for patients that include functional goals Monitor patients’ progress and modify treatment plans Maintain accurate clinical records and complete required reports Attend clinics, interdisciplinary case conferences, staff meetings, and in-service training Evaluate for and order adaptive and durable medical equipment and instruct patients and caregivers in its use Consult with caregivers and any other agencies involved in the care of assigned patients Participate in the IEP process Provide general direction to Therapy Aides, volunteers and student interns Bilingual: Spanish would be great, but not required If client wants to interview, they will have us coordinate in-person interviews for local candidates, or virtual for travel candidates.
