THIS IS A HOME HEALTH NEED! 
 

• The physical therapist is responsible for providing physical therapy services to clients of the agency. The primary function is the provision of direct patient care, including collaboration with other team members and participation in the development of the patient’s comprehensive plan of care.
• Rehabilitation equipment including but not limited to:
  • splints,
  • orthoses
  • prostheses,
  • wheelchairs,
  • ADL equipment
  • Modalities.
  • Candidate required to be Home Health Certified
• Submission Requirements
• 2+ acute nursing care (hospital base) and Home Care experience [required]
• Valid Driver License [required] for this need
• Must be able to work independently with minimal supervision/assistance
• Acute care experience [preferred]
• Certified home health experience including proficiency with OASIS required and NON-NEGOTIABLE [REQUIRED]
   
• AT LEAST TWO SPECIFIC INTERVIEW DATES/TIMES OF AVAILABILITY FOR INTERVIEW MUST BE ADDED TO TRIO/ PROFILE - PROFILES WILL NOT BE PRESENTED WITHOUT THIS INFORMATION.  "ANYTIME" WILL NOT BE ACCEPTED.
