Description

Responsibilities

Perform comprehensive patient assessments
Coordinate care and manage discharge planning
Organize and participate in family meetings
Collaborate on interdisciplinary treatment plans
Arrange DME, SNF placements, and ambulance transportation independently
Work with HMOs, IPAs, and community resources
Support patients with:
- Mental health needs
- Substance abuse treatment coordination
- Homelessness resources
- Advance directives, POLST, and protective services

Requirements & Qualifications

Active CA RN License (Pending accepted but must be active by start date)
Minimum 2 years of acute medical Case Manager experience
Proven experience in care coordination
Epic EMR experience REQUIRED
Prior travel experience REQUIRED
Trauma Level I or II experience preferred

Work Setting: Acute Care Hospital Case Management / Care Coordination Patient Ratio: 1:25 Patient Population: Adults & Geriatrics Floating required to Laguna Facility
