HOME HEALTH RN CASE MANAGER

DESCRIPTION:

Job Title: Experienced Home Health RN – Central Vermont

Location: Barre, VT 05641

Schedule: 3 days per week, 12-hour shifts

Position Type: Contract

Responsibilities:

Provide skilled nursing care in patients’ homes
Perform assessments and develop individualized care plans
Complete OASIS documentation accurately and timely
Manage patient caseloads efficiently, coordinating with the interdisciplinary team
Educate patients and families on disease management, medications, and health maintenance

Qualifications:

Valid RN license in Vermont
Proven home health and case management experience required
OASIS experience required
Strong preference for nurses with travel experience
Must be available for the full duration of the contract with no requested time off
