Description

Shift: Day | 5x8-Hour Hours: 08:00 AM – 05:00 PM

Requirements:

• 1 year of experience required (2 years preferred)
• Must be an independent practitioner
• Home Health experience required; OASIS training preferred
• Must have a valid driver’s license and reliable vehicle for home visits
• BLS certification
• Active State License

Work Details:

• Skilled Home Care (Medicare, Medicare Advantage & Commercial Insurance)
• 6–8 home visits per day
– Minimum 6 points/day (may increase by manager’s discretion)
• Equipment provided: Laptop, MiFi (internet access), Portable Printer

Patient Population:

• Med/Surg, Cardiac, Ortho
• Post-surgical (hips, knees, shoulders)
• COPD, CHF, Pneumonia
• CVA, Falls, Weakness
• Parkinson’s, Failure to Thrive
• Respiratory & Neurological patients
• Trachs, Trauma
• AKA/BKA
• ADLs
