*Requirements:
*MS or Compact state license required
*Shift: 8a-4:30p with rotating weekend shifts
*Locals are not allowed/ travelers must be greater than 60 miles away from the facility
*BLS required
*Must have RECENT experience in both discharge planning and UR in hospital, 3+ years of Acute Hospital Case Management experience
*Cerner is our EHR, CM uses MorCare for documentation and CarePort for referral system
Facility is looking for a hospital Case Management experience of at least 3 years. Not a charge nurse, not a staff nurse, not an outpatient home health or hospice case manager. Candidates have to be able to hit the ground running as an acute, hospital case manager on day 2. That means knowing all the things about discharge planning to send a patient to home health, SNF. LTAC, IRF. How to do utilization review for medical necessity using MCG criteria. Know the rules of how to provide patient choice letters and IMMs as required by the Conditions of Participation and how it is documented.
