Occupational Therapist (OT) – Home Health | New Haven, CT

  • Developing and implementing individualized treatment plans
  • Collaborating with PTs, SLPs, nursing, and other team members as needed
  • Completing timely and compliant documentation
  • Educating patients and families on adaptive strategies, safety, and therapeutic exercises
