Providing skilled PT services in patients’ homes to improve mobility, strength, and functional independence
Developing and implementing individualized treatment plans
Collaborating with OT, SLP, nursing, and other team members as needed
Completing timely, accurate, and compliant documentation
Educating patients and families on mobility, safety, and therapeutic strategies
