Required skills: Assessment, Cardiopulmonary/Circulatory, Documentation, Equipment and Devices (Gait belt, Rolling Walker, Sit to stand lift, Sliding boards, Soft/Hard collars, Standard Walker, Supplemental Oxygen, Upper extremity prosthetics, Wheelchair Assessment), Ortho, Neuro, Home Exercise Program, Medications, Patient family education, Signs & Symptoms physical, sexual, psychological abuse, Safety and injury prevention, Billing
Must have home health experience; OASIS training preferred
Certs required: BLS, state licensed
Scrubs: Any solid color scrub
Unit Info (share with your traveler):
Patients: med/surg, cardiac, ortho, hips/knees/shoulders, post-surgical, copd, chf, pneumonia, CVA, Falls, weakness, parkinson's, failure to thrive, resp patients, neuro, trachs, trauma, AKA, BKA, ADL'S
Skilled Home Care (Medicare, Medicare Advantage and Commercial ins.)
Home skilled visits - 6-8 per day (MINIMUM of 6 points per day at managers discretion may increase by 1 point)
1-2 Weekends per contract
Laptop, MiFi for internet access, Portable printers
BayCare Forms, BayCare Scheduling, Ace Timesheet documentation system
Area: Davenport/Haines City and Lake Wales/Winterhaven

Other requirements:
1 year experience required
Must have valid drivers license and car to make home visits.
Must be COVID vaccinated or with exemption form on file.
Must have flu shot or declination and in both cases mandatory mask wearing at all times while at work.
Based on the needs of the HomeCare office, the visits and hours will be self-scheduled
