Job Title<ul><li>Therapist - Physical Therapist</li></ul>Location<ul><li>Beaver Dam, WI</li></ul>Schedule / Job Type<ul><li>Full-time, Days shifts, Self-scheduling, 8:00 AM - 4:30 PM</li></ul>Key Responsibilities<ul><li>Provide outpatient, acute inpatient, skilled nursing, and home health physical therapy services.</li><li>Manage an average of 5 patients per day, including evaluations and visits, with flexibility depending on patient needs.</li><li>Utilize skills in OASIS documentation, with Lymphedema certification considered a plus.</li><li>Collaborate with Rehab Aides and supervised by licensed PTs to deliver comprehensive care.</li><li>Follow department-specific orientation including computer-based training and shadowing.</li></ul>Required Qualifications<ul><li>Valid BLS certification.</li><li>Experience with OASIS documentation.</li><li>Ability to treat geriatric patients with diagnoses such as joint replacements, stroke, CHF/COPD, and Parkinson’s.</li><li>Willingness to work in home health settings and cover assigned areas.</li></ul>Preferred Experience / Skills<ul><li>Lymphedema certification.</li><li>Prior experience in outpatient or home health rehab.</li></ul>
