Requirements: Graduation from an accredited practical nursing program, current NYS LPN license, and 1 year of clinical experience preferred.
Under the supervision of an RN or physician, the LPN provides direct patient care by assessing, treating, and monitoring patients while applying sound clinical judgment. Responsibilities include accurate documentation and re-evaluation of patient conditions, contributing to individualized care plans, monitoring outcomes, and coordinating discharge needs with the care team. The role requires close collaboration with interdisciplinary staff, adherence to patient privacy and safety standards, and the ability to perform physical duties including prolonged standing, assisting with patient mobility, and potential exposure to blood and body fluids.
