Position Overview:
We are seeking a dedicated and compassionate Physical Therapist (PT) for a contract opportunity in a Skilled Nursing Facility (SNF) setting. This role offers a 13-week contract with a guaranteed 30 hours per week, ideal for clinicians looking for stability and flexibility while making a meaningful impact on patient recovery and mobility.

Key Responsibilities:

• Perform comprehensive physical therapy evaluations and assessments
• Develop and implement individualized treatment plans
• Provide therapeutic interventions to improve mobility, strength, balance, and functional independence
• Document patient progress and treatment outcomes accurately and timely
• Collaborate with interdisciplinary healthcare teams to ensure optimal patient care
• Educate patients and caregivers on treatment plans and home exercise programs

Qualifications:

• Active California Physical Therapist license (required)
• Experience in a Skilled Nursing Facility preferred, but new graduates are welcome to apply
• Strong clinical, communication, and organizational skills
• Ability to work independently and as part of a collaborative care team
