Job Summary

We are seeking a licensed Physical Therapist (PT) for a contract assignment in a Skilled Nursing Facility. The Physical Therapist will evaluate patients, develop and implement treatment plans, and help individuals restore function, improve mobility, and reduce pain through evidence-based physical therapy interventions.

Responsibilities

• Evaluate patients’ physical abilities and functional limitations
• Develop and implement individualized physical therapy treatment plans
• Provide therapeutic exercises, manual therapy, and mobility training
• Monitor patient progress and modify treatment plans as needed
• Educate patients and caregivers on exercises, mobility, and safety
• Collaborate with an interdisciplinary healthcare team
• Document evaluations, treatments, and patient outcomes accurately and timely
• Ensure compliance with state regulations and facility policies

Requirements

• Active state Physical Therapist license (must be current)
• Less than 1 year of experience accepted
• Strong clinical assessment and documentation skills
• Ability to work independently and in a team-oriented environment
