Position Overview

• Location: Hallock, MN
• Setting: OP; SNF/LTC; limited HH & IP
• Pay Package: Starting at $2600/week
• Guaranteed Hours: 40-hour weekly guarantee
• Schedule: Flexible; Monday–Friday, 8:00 AM–4:30 PM (No weekends, holidays, or call)
• Contract Length: 13 weeks
• Start Date: ASAP

Qualifications

• Graduate of an approved school of Physical Therapy, with a minimum of a bachelor's degree
• Valid state PT licensure (or currently in a PT educational program)
• CPR certification
• Valid driver’s license
• Must successfully pass a background check upon hire
• Strong communication and interpersonal skills
• Experience in SNF and/or outpatient preferred — new grads encouraged to apply!

Responsibilities

• Provide supervision for Physical Therapist Assistants and Therapy Aides in accordance with state/federal regulations
• Evaluate patients, review medical history, and diagnose movement dysfunction
• Develop and modify individualized treatment plans
• Teach therapeutic exercises and provide hands-on techniques to support healing
• Maintain accurate documentation and patient progress records
• Educate patients and families on home programs and recovery expectations
• Perform administrative tasks including reports, communication, and documentation per HIPAA
