Unit Overview Unit Type: Cardiac Stepdown / Cardio-Thoracic # of Beds: 24 Nurse:Patient Ratios: Days: 1:4 Nights: 1:4–5 Required Certifications: Mandatory: BLS, ACLS Preferred: NIHSS Common Diagnoses / Patient Types: Pre/post cardiac procedures: stents, pacemakers, ICDs CABG (≤24 hrs post-op) Cardioversions on the floor Thoracotomy (direct from PACU) Vascular procedures NSTEMI patients Tracheostomy patients (home vented or bipap/cpap) Chest tubes Pulling pacer wires (usually ICU-level skill) Experience / Must-Have Skills: Strong experience with pre/post cardiac patients Pre/post stents, pacemakers, ICDs Pre/post CABG and vascular procedures Drips & Monitoring Titration: Some drips titrated by RN; dobutamine & dopamine usually non-titratable. Common Drips: Cardizem, Amiodarone, Dopamine, Dobutamine, Nitro, Integrelin Monitoring: Tele/Monitor Techs on unit RN does not need to read strips independently Travelers don’t need advanced telemetry expertise Staffing & Support Charge Nurse / House Supervisor: Day: Charge does not take patient assignments Night: Charge may take 1–2 patients ARR Nurse (Rapid Response) available Nurse Aides: Day: 1:8 ratio Night: 1:12 ratio Respiratory Therapy / Equipment: Bipap, Cpap, home vents (home vent experience not required) Scheduling & Shifts Shift Times: 6:45 – 18:45 (12-hr shifts) Scheduling Options: Block scheduling allowed, self-scheduling allowed Weekend Requirements: Every other weekend or 4 shifts/month (flexible) Holiday Requirements: Every other holiday; main holidays: New Year, 4th of July, Thanksgiving, Christmas, Memorial Day, Labor Day Call/Standby: None required Floating: Possible, with permanent staff rotation Orientation & Additional Info Unit-Specific Orientation: 2–3 shifts Scrub Color / Dress Code: Navy blue scrubs (provided) Additional Notes: Heavy cardiac/CTICU unit High acuity surgical patients Preference for stepdown-experienced nurses vs telemetry-only nurses
