Description

Registered Nurse – Case Manager (Contract)

West Hollywood, CA
Day Shift | 5x8 (8:00 AM – 5:00 PM)
13-Week Contract

Key Responsibilities

Perform inpatient case management and utilization review
Apply InterQual and Milliman criteria to assess medical necessity
Coordinate patient care across acute, post-acute, home health, and SNF settings
Develop, implement, and update individualized plans of care
Facilitate safe and timely discharge planning
Collaborate with physicians, social services, and transportation teams
Ensure compliance with CMS, HIPAA, Joint Commission, and NCQA standards
Document accurately and efficiently in Epic

Required Qualifications

Active California RN License (Required)
Minimum 2 years of inpatient RN Case Management experience
Strong working knowledge of HMO and Medicare for inpatient, home health, and SNF settings
Epic experience strongly preferred
BLS Certification
