Shift: Mid 5x8-Hour (14:00 - 22:00)

Position Summary:

The Licensed Practical Nurse (LPN) provides compassionate, patient-centered nursing care to residents in the Moore Place Dementia Care unit. This role involves delivering skilled nursing care to residents with Alzheimer’s disease, dementia, and neurobehavioral conditions, ensuring their safety, comfort, and dignity. The LPN works closely with the interdisciplinary team to support individualized care plans and promote a therapeutic, structured environment..

Responsibilities & Duties:

• Provide direct nursing care to residents with dementia and neurobehavioral conditions in accordance with facility policies and procedures.
• Administer medications safely and accurately, including psychotropic medications, and document appropriately.
• Perform clinical assessments, monitor vital signs, and report changes in condition promptly.
• Provide assistance with ADLs (activities of daily living) such as bathing, dressing, grooming, and feeding.
• Implement individualized care plans, behavior support plans, and de-escalation techniques.
• Monitor and manage behavioral symptoms, including agitation, wandering, anxiety, and aggression.
• Maintain a safe and therapeutic environment, ensuring resident safety and fall prevention.
• Collaborate with the interdisciplinary team (RN, Social Worker, PT/OT, Activities, Dietary, etc.).
• Provide supportive communication with families and caregivers.
• Document resident care accurately in the medical record.
• Maintain infection control standards and adhere to facility policies.
• Participate in training, staff meetings, and ongoing education.

Requirements:

• Years of Experience: 2 year

• BLS from AHA
