Description

RN Case Manager – Acute Care | Travel Contract

Apple Valley, CA
13-Week Contract

Day Shift | 5x8 (08:00 AM – 4:30 PM)
Experienced Travelers Only

Responsibilities

Perform comprehensive patient assessments and needs evaluations
Coordinate care and discharge planning for assigned caseload
Manage utilization review and prior authorizations
Collaborate with physicians, social services, PT, and interdisciplinary teams
Arrange DME, SNF placement, and ambulance transportation independently
Participate in family meetings and treatment plan development
Ensure compliance with CMS, Joint Commission, DRG, and regulatory standards
Document care coordination activities in Epic

Requirements

Active CA RN license (pending accepted if active by start date)
2+ years Acute Care Case Management / Discharge Planning experience
Hospital-based experience REQUIRED (Behavioral Health does NOT qualify)
Prior travel experience REQUIRED (hard stop)
Epic charting experience
Strong knowledge of utilization review, care coordination, and discharge processes
