🏥 Case Manager – Critical Need (RN)

📍 Atlanta, GA
🏢 Kaiser Foundation Health Plan – Regional Office (9 Piedmont Center)
🕘 Schedule: Monday–Friday | 8:30 AM – 5:00 PM
⏱ 40 hrs/week | 8-hr shifts

✅ Requirements

• Active RN license (Compact or Georgia license preferred)
• BLS certification (AHA)
• Minimum 2 years of RN experience
• Strong understanding of case management, care coordination, and utilization review principles
• Experience working in a managed care, hospital, or large healthcare system
• Proficiency with electronic medical records and case management platforms
• Excellent communication, organization, and critical-thinking skills

🩺 Key Responsibilities

• Coordinate patient care across the continuum, including inpatient, outpatient, post-acute, and community services
• Perform utilization review to ensure appropriate level of care, length of stay, and resource utilization
• Develop, implement, and evaluate individualized care plans in collaboration with physicians and interdisciplinary teams
• Lead discharge planning and transitions of care to promote safe, timely discharges and reduce readmissions
• Communicate effectively with patients, families, providers, and external agencies regarding care plans
• Ensure compliance with Kaiser Permanente policies, regulatory requirements, and evidence-based guidelines
• Document all case management activities accurately in electronic systems
• Participate in quality improvement initiatives and care coordination meetings
