Description
Job Title: Social Worker (LCSW / LMSW) – Hybrid
Location: Bronx, NY
Job Type: Full-Time
Schedule: Hybrid – Onsite once weekly (as needed)
Position Overview
We are seeking dedicated Social Workers (LCSW or LMSW) to join our outpatient team in the Bronx, NY. This hybrid position focuses on providing high-quality clinical and psychosocial services to diverse patient populations, emphasizing coordinated, patient-centered care in a collaborative healthcare environment.
Key Responsibilities
- Conduct comprehensive clinical and psychosocial assessments
- Develop, implement, and manage individualized treatment and care plans
- Provide counseling, crisis intervention, and ongoing clinical support
- Monitor client progress and update care plans as needed
- Collaborate with physicians, nurses, behavioral health providers, and interdisciplinary teams
- Assist patients and families with access to community resources and social services
- Support care coordination for patients with Medicaid and Medicare coverage
- Maintain timely, accurate, and compliant clinical documentation
- Coordinate with community partners to ensure continuity of care
Qualifications & Requirements
LCSW / LMSW Candidates:
- Master’s degree in Social Work
- Active LCSW / LMSW license in New York State
- Minimum 1 year of professional experience
- Bilingual English/Spanish required
- Strong clinical assessment and communication skills
- Comfortable working in a hybrid care environment
