Job Summary
The Speech-Language Pathologist will provide evaluation and treatment services to SNF patients, working collaboratively with an interdisciplinary care team to support patient-centered rehabilitation outcomes.
Key Responsibilities
- Evaluate and treat patients in a Skilled Nursing Facility setting
- Develop and implement individualized treatment plans
- Collaborate with nursing, therapy, and medical teams
- Complete timely and accurate documentation per facility standards
- Utilize Net Health for documentation
Requirements
- Active California SLP License (required)
- SNF experience preferred (mentorship available if experience is limited)
- Strong documentation and time-management skills
- Flexible, collaborative, and patient-focused approach
- Ability to float to a sister facility within walking distance if needed
