Speech-Language Pathologist (SLP) – Skilled Nursing Facility
Contract | Quick Start
We are seeking a qualified Speech-Language Pathologist (SLP) to join a Skilled Nursing Facility on a contract basis. This role is ideal for an SLP with prior SNF experience who is confident working in a fast-paced rehabilitation environment.
Position Overview
The Speech-Language Pathologist will evaluate, diagnose, and treat residents with communication, cognitive, and swallowing disorders, collaborating closely with the interdisciplinary rehab team to support patient-centered care.
Responsibilities
- Evaluate and treat speech, language, cognitive, and swallowing disorders
- Develop and implement individualized treatment plans
- Document evaluations, progress notes, and discharge summaries per facility standards
- Collaborate with PTs, OTs, nursing staff, and physicians
- Ensure compliance with all clinical, regulatory, and documentation requirements
Requirements
- Active California Speech-Language Pathologist license
- Minimum 1 year of Skilled Nursing Facility (SNF) experience
- Experience with Rehab Optima / Net Health preferred
- Strong clinical judgment, documentation, and time-management skills
- Ability to work independently and as part of a multidisciplinary team
