Description
Minimum Years of Experience required?
- 2 years of recent acute inpatient social worker/case management experience
- 4 years overall social work experience
Will you accept a first-time traveler? No
Charting: EPIC, exp required
Required License, National Certification, Certs: Master s in social work and WA state License LICSW is required BLS not required
Skills required: Acute care case management experience is a must Behavioral health assessment, diagnosis, treatment planning, discharge planning for post-acute placements (understanding SNF level of care, understanding regulatory requirements)
Support within the Department:
- 2-3 total social workers, 1 other PRN SW Case Management RNs Management Will have CM Assistant soon
Patient Types/Common Diagnoses:
- Inpatient and ER patients. Will mostly see adults but could see some infants and children. Will see ICU, PCU, MedSurg, ER and LD patients
